The 1,25-dihydroxy vitamin D test measures calcitriol, the active hormonal form of vitamin D made in the kidneys. It regulates calcium and phosphate balance. It is not used to check vitamin D deficiency; the 25-OH vitamin D test is used for that.
Normal levels are roughly 18–78 pg/mL, varying by lab.
Low levels suggest chronic kidney disease, hypoparathyroidism, or vitamin D-dependent rickets. High levels may indicate sarcoidosis, tuberculosis, lymphoma, or hyperparathyroidism, which can cause high blood calcium. The test is useful in evaluating unexplained calcium disorders.
